I have uploaded updated printable STARS forms reflect the March 9, 2024 enhancements to the STARS data system. Booz Allen Hamilton created five types of STARS forms to aid in data entry: 

1) Beneficiary Contact Form; 

2) Group Outreach and Education Form; 

3) Media Outreach and Education Form; 

4) Team Member Form; 

5) Training Form. 

These forms can be helpful for any AAA that needs a way for staff to collect data on paper for later entry into STARS.  These can be printed and filled by hand or digitally filled out at your convenience.  You may also forward the forms to the Medicare-eligible beneficiary for them to fill out and submit back to you, with reporting in STARS online completed at a later time.  If you have any issues accessing or viewing the files below, just contact me at Jessica.Ross@la.gov or via phone or text at 225-328-4595.
